Transitional care helps older adults recover safely and comfortably at home following a hospital stay, surgery, illness, or rehabilitation. This critical phase of care focuses on preventing readmission, managing medications, and supporting activities of daily living. 

Biscochito’s professional caregivers are trained to work collaboratively with your healthcare team. Whether you need 24/7 support or a few hours a day, our goal is simple: help you heal while honoring your rhythms, routines, and dignity.
